Apply For Spay/Neuter

It's easy to apply for low cost spay/neuter through Fix Them All.  Just follow these simple steps:

1.  First, go to the Schedules page.  This summarizes your options and gives dates of upcoming clinics and transports.

2.  Second, learn more about our spay/neuter providers, including their locations, requirements, extra charges and optional services, on our Veterinarians page.

3.  To apply and make your copayment online, complete and submit the form below.  Your co-pay is $50 per cat and $75 per dog.  

To apply and pay by mail, print and complete this application form and send it with a check or money order made out to "Fix Them All" to:  Fix Them All, 7257 NW 4th Blvd. #20, Gainesville, FL 32607.

Application for Spay/Neuter

Please submit one form per pet.  Limit two cats per family per event.

Your co-payment will be $50 for a cat or $75 for a dog.  

Please allow 3-5 days for your application and co-payment to be processed.  For clinic appointments, a surgery certificate will be emailed to you for the participating clinic of your choice.  If no surgery date is provided, contact the clinic to schedule the appointment and notify us of the surgery date.   For transport programs, an email will be sent to you with your transport date and instructions.

Co-payments are non-refundable.  However, with proper notification, appointments can be rescheduled up to 60 days. No-shows and late cancellations prevent other pets from getting fixed.  If you do not contact us prior to surgery day to cancel your pet's surgery, you co-payment could be forfeited.

Check the list of our participating veterinarians and select one as your preferred veterinarian.  If you have no preference, select "Please chose a vet for me".  Do not contact the veterinarian until you receive confirmation from us.  Please note that unless you bring proof of current rabies vaccination, a vaccination will be required on day of surgery.
